Claim Documents

Please download and submit the correct forms. The ‘Liquidation Claim Documents’ must be used for all 

Company and Close Corporation Estates. For Personal (Individual) Estates use the ‘Sequestration 

Clam Documents’. Kindly note that The Master of the High Court does NOT accept faxed or scanned 

documents, so we require the originals. If you be unsure on how to complete these Claim Documents,

Nomination Forms

 2) The amount owing must be completed in both ‘numbers’ and ‘words’. 

 3) If the Creditor is a Company, the Nominantion Form must be signed by a Director. 

 4) If the Creditor is a Close Corporaition, the Nominantion Form must be signed by a Member.
